Cibinetide, the other name for Ara-290, is a synthetic peptide designed from a segment of erythropoietin, the natural driver behind the body's production of red blood cells; scientists changed it so that it flips on a different pathway instead, one linked to repairing tissue. It has gone through several small human Phase 2 trials, mostly in people with nerve damage from sarcoidosis or diabetes, and carries FDA Orphan Drug status for a few of those conditions, though it is not an approved medicine.
What it is
Ara-290 is an 11-amino-acid peptide built from a piece of erythropoietin's structure. Erythropoietin normally binds a receptor that increases red blood cell production, but researchers redesigned Ara-290 to activate a different target, sometimes called the innate repair receptor, that is linked to tissue protection and repair rather than blood cell production [4]. In lab studies, the compound has not raised red blood cell counts the way erythropoietin does.
Most of the human research has focused on small-fiber neuropathy, damage to the thinnest nerve fibers in the body, in people with sarcoidosis, an inflammatory disease, or diabetes. Researchers have measured nerve density in the surface of the eye using a specialized microscope as one way to see whether nerve fibers are regrowing, alongside patient-reported pain and sensation tests [1][3].
How it is studied
Human studies have been small, randomized, placebo-controlled Phase 2 trials, typically giving the peptide by injection under the skin daily for around 28 days to a few dozen patients, with follow-up assessments weeks later [1][3]. Laboratory work has separately tested the compound on isolated human and mouse pancreatic islet cells and on immune cells called macrophages, exposing them to inflammatory stress in a dish to see whether the peptide protects them [2].
What studies report
- In a placebo-controlled trial of sarcoidosis patients with small-fiber nerve damage, 28 days of the peptide was linked to a measurable increase in nerve fiber density in the surface of the eye, alongside improved pain-threshold tests, with benefits still present sixteen weeks later. [1]
- In a larger follow-up trial testing several amounts in the same condition, the 4 mg daily amount produced roughly a 23 percent increase in that same eye-surface nerve fiber measure compared with placebo. [3]
- In isolated pancreatic islet cells and immune cells exposed to inflammatory stress in a dish, the peptide reduced cell death and lowered the release of inflammatory signaling proteins. [2]
What is not known
All human trials of Ara-290 published so far have been small, in the range of a few dozen to around sixty participants, and none has reached the scale of a pivotal Phase 3 trial. A separate small trial in diabetic eye disease, testing the compound for a different reason, did not show benefit on the eye measures it was designed to improve, showing that the nerve-repair signal seen in sarcoidosis studies has not automatically carried over to every condition tested.
Reported adverse findings
- The sarcoidosis and diabetic-neuropathy trials describe the compound as generally well tolerated, with no drug-targeting immune antibodies detected and no signs of the increased red-blood-cell production seen with erythropoietin. [1][3]
Regulatory status
No regulator, including the FDA and Health Canada, has approved Ara-290 as a medicine. The FDA has granted it Orphan Drug status for a small number of rare conditions, a designation that offers development incentives but is not itself a marketing approval.
